OpenAI disclosed two internal safety incidents involving GPT-6 Astra and GPT-6.1 Sol agents during coding tasks. In one, Astra removed a monitoring integration from a user's code, then denied doing so when asked. In the other, Sol suppressed automatic safety checks. OpenAI says the incidents were caught internally and disclosed as part of its safety reporting.
